Title: Aural haematoma? Motley's ear has gone wonky!

Sarah_Pussycat - January 4, 2009 11:22 PM (GMT)
Motley has suddenly developed a wonky ear! I had a closer look and there was a huge lump on it that sort of makes the top bit flop over.... It's only happened today.

I was REALLY worried at first, but after a bit of googling I came across these haematoma things that look EXACTLY like what's going on here! It's when a blood vessel bursts inside the ear - usually due to ear mites (I haven't noticed Motley scratching, but Ruby had them earlier in the year so that's a possibility - also maybe one of her play-fights with Sparkle might have got out of hand).

She's definitely going to the vets in the next couple of days - I was just wondering if anyone else has any experience of this and how it was treated?

Crystal - January 4, 2009 11:56 PM (GMT)
Never heard of it - googled it...found this...

QUOTE
There are two aspects to treatment, dealing with the haematoma itself and dealing with the underlying cause of the ear irritation.
Firstly, surgery will be performed to remove the blood clot and the space left between the skin and cartilage using special sutures. This will prevent another haematoma forming.
Secondly the cause of the ear irritation must be diagnosed and treated otherwise the problem may reoccur. The most common cause of ear irritation is an ear mite infestation. Other causes include:- ectoparasites, allergic diseases, masses, polyps or foreign bodies in the ear canal.
QUOTE
There are 2 steps for successful treatment -

1. The fluid needs to be removed from the pinna  This can be done in 2 ways.
Small haematomas can be treated by aspirating the fluid via needle and syringe whilst the pet is under sedation, (it is often possible to do the procedure conscious in cooperative animals).  We usually insert an small amount of anti-inflammatory solution into the space where the blood was to prevent further accumulation.  This is successful in about 80% of cases.  If the haematoma reforms then the procedure can be repeated.
Larger, or recurrent smaller haematomas, need to be incised with a scalpel and stitched.  This is done under a general anaesthetic.  A small piece of skin is removed from the underside of the pinna and the haematoma fluid drained away.  Several sutures are then placed through the pinna to obliterate the space where the blood had accumulated. Since the skin over the haematoma has been pushed away from the cartilage, it must be reattached to it to prevent another haematoma forming.
2. The cause of the problem needs to be diagnosed and treated.
cats with aural haematomas often have ear infections that has caused them to shake their head, or scratch their ears. Any infection present will need medication to treat it; usually in the form of ear drops.  Some cats have no infection but have foreign material (wax, grass seeds etc) lodged in the ear canal.  The ear canals will be cleaned out and any foreign material removed.

Definately keep us updated - I hope she's alright!

Thanks - she seems ok in herself, she's still acting the same and isn't off her food or anything, and the lump doesn't seem to be causing her any pain. She's letting me look at it and feel it anyway!

She just looks a bit like a gremlin on one side!

ElfinErv - January 5, 2009 01:52 AM (GMT)
yup one of my cats had a Aural haematoma...had to be fixed with a bit of surgery otherwise ear damage could result

Sarah_Pussycat - January 5, 2009 05:52 PM (GMT)
She's booked in with the vet tomorrow! :)

Here's a photo I took - notice how she's holding her ear out to the side, the swelling is on the part near the middle of her head and it's making the top part of her ear flop over.

We're back from the vets!

I was right, it IS a haematoma, and she's had an injection to help any swelling for now. The vet said that as it's fairly recent there's no point trying to drain it now as they normally just fill back up again!

He couldn't find any obvious cause and her ears looked fine apart from the obvious (could possibly be a play fight that got a bit out of hand) but I mentioned that Ruby had the ear mites so I've got some stuff just in case to do ALL the cats with.

Motley's booked in next week to have the haematoma drained - the vet said she'll probably be ok without sedation as she was so placid (this is the same cat that normally scares grown men lying there purring away on the table - she LOVES vets!).

:)

Melanie - January 6, 2009 10:53 PM (GMT)
Glad to hear its nothing too serious!

Elizabeth - January 7, 2009 09:12 PM (GMT)
Glad she's been taken care of!! We've had dogs come into the clinic with haematomas.

Sarah_Pussycat - January 7, 2009 10:58 PM (GMT)
It actually looks a bit better tonight - the anti-inflammatory injection must have helped a bit! :)

She STILL looks half-gremlin though - but it quite suits her! :rotfl:

Crystal - January 8, 2009 04:30 PM (GMT)
Awesome!

Keep us updated. :D

Allie - January 8, 2009 05:37 PM (GMT)
I hope Motely's ear will be to normal soon.
Now that I think about it my first cat Tibby had gotten an abscess at the base of his tail. It was swollen to the point the vet put a draining tube thru the abscess. He had gotten in a cat fight outside. I remember the tube and his shaved tail, this was the early 80s. I was gross...sounds like Motley's issue is easier to treat.

Good news! :) I giggled at the Gremlin comment... sorry! :giggle:

Sarah_Pussycat - January 13, 2009 08:37 PM (GMT)
We've just got back from having Motley's ear drained! She didn't need sedation, and only hissed ONCE. :)

Then the vet washed her ears out which she quite enjoyed! She MAY have to go back for surgery - we just have to wait and see if it fills up again (fingers crossed!).

Here she is showing off her newly-drained ear - it's all floppy now as it's been all filled with blood for the last week. She looks a bit scruffy at the moment - that's the stuff the vet was using to clean her ear.

Sarah_Pussycat - January 14, 2009 01:22 PM (GMT)
It's filling up again
So I called the vets and she's having the op on Friday so I can be at home with her all weekend while she recuperates :)

Melanie - January 14, 2009 02:07 PM (GMT)
Too bad that it has to come down to an operation but at least its nothign serious!

TxnKats - January 14, 2009 02:11 PM (GMT)
Poor girl. We've only had one hematoma come through our practice that did fine with just draining. Most tend to need surgery. Her ear will probably always look off from now on, but once she has the surgery she should be good to go.

Elizabeth - January 14, 2009 02:54 PM (GMT)
Aw, poor Motley! She'll do fine, though. Wishing her a speedy recovery from her surgery this Friday! Be good Motley. :nono: :rotfl:

Sarah_Pussycat - January 14, 2009 10:22 PM (GMT)
When Motley's all stretched out by the radiatior with her head back her ear goes almost straight!

Just took this pic - you can see the lump (it's not QUITE as big as it was before she had it drained and has a ripple in the middle of it now!) Look closely at the ear that's NOT next to the carpet!

Oh no, I totally missed this!

I'm glad she's okay - poor thing, I hope she deals with e-collar okay. :(

Lots of good thoughts coming from here - I hope she heals up nicely. I'm glad she got everything done at once, though, and hopefully it's behind her now.

Gorgeous little thing. :luv2:

Sarah_Pussycat - January 16, 2009 11:00 PM (GMT)
:rotfl: Motley has now worked out how to stretch out by the radiator in the collar!

She just hasn't worked out how to go up stairs in it - she can go DOWN ok, but I have to carry her up! :)

I think Motley looks more like she wants to smash Sarah with something. :rotfl: She doesn't look very happy.

Sarah_Pussycat - January 20, 2009 09:16 PM (GMT)
Another quick update!

Motley was back at the vets for a check-up yesterday - the vet says she's healing better than most cats do! Her temperature and heart rate were just PERFECT too!

She has to go back a week on Wednesday to have her stitches out - she's doing great in the collar though and hasn't managed to get it off since I tightened it up a bit!

:)

Crystal - January 20, 2009 09:29 PM (GMT)
Awesome!!!

Sarah_Pussycat - January 20, 2009 09:34 PM (GMT)
